recently launched the PM Family Care Tracker (PM-FCT) as a pilot project in Gujarat. <\/span><\/p>\n<h2><b>About PM Family Care Tracker (PM-FCT)<\/b><\/h2>\n<ul>\n<li style=\"text-align: justify;\"><span style=\"font-weight: 400;\">It is a <\/span><b>digital platform <\/b><span style=\"font-weight: 400;\">aimed at <\/span><b>strengthening maternal and child health, nutrition, and family welfare<\/b><span style=\"font-weight: 400;\">.<\/span><\/li>\n<li style=\"text-align: justify;\"><span style=\"font-weight: 400;\">The digital platform would ensure that <\/span><b>pregnant women, mothers, and children receive all eligible government benefits<\/b><span style=\"font-weight: 400;\"> through <\/span><b>continuous monitoring<\/b><span style=\"font-weight: 400;\"> and timely intervention.\u00a0<\/span><\/li>\n<li style=\"text-align: justify;\"><span style=\"font-weight: 400;\">It has been designed to <\/span><b>digitally monitor every stage of a child&#8217;s development<\/b><span style=\"font-weight: 400;\">, from <\/span><b>pregnancy through 18 years of age,<\/b><span style=\"font-weight: 400;\"> while helping eligible families receive welfare benefits on time.\u00a0<\/span><\/li>\n<li style=\"text-align: justify;\"><span style=\"font-weight: 400;\">The PM-FCT provides <\/span><b>end-to-end monitoring of key health milestones<\/b><span style=\"font-weight: 400;\">, including antenatal and postnatal care, immunisation, nutrition, growth monitoring,<\/span><b> school enrolment and attendance,<\/b><span style=\"font-weight: 400;\"> and <\/span><b>adolescent health services.\u00a0<\/b><\/li>\n<li style=\"text-align: justify;\"><span style=\"font-weight: 400;\">The platform also features<\/span><b> digital Health Passports for individuals and families, dashboards<\/b><span style=\"font-weight: 400;\"> to<\/span><b> facilitate delivery of government welfare schemes,<\/b><span style=\"font-weight: 400;\"> and <\/span><b>automated alerts<\/b><span style=\"font-weight: 400;\"> f<\/span><b>or missed vaccinations and other essential health services.\u00a0<\/b><\/li>\n<li style=\"text-align: justify;\"><span style=\"font-weight: 400;\">For Example:<\/span>\n<ul>\n<li style=\"text-align: justify;\"><span style=\"font-weight: 400;\">Alerts will be generated automatically if a child misses vaccination or drops out of school.\u00a0<\/span><\/li>\n<li style=\"text-align: justify;\"><span style=\"font-weight: 400;\">Notifications will reach local officials, legislators and Members of Parliament, enabling volunteers and government authorities to intervene promptly and ensure that every child receives essential services.\u00a0<\/span><\/li>\n<\/ul>\n<\/li>\n<li style=\"text-align: justify;\"><span style=\"font-weight: 400;\">It is expected to enable <\/span><b>timely interventions<\/b><span style=\"font-weight: 400;\">, <\/span><b>reduce maternal and infant mortality, <\/b><span style=\"font-weight: 400;\">address malnutrition, and minimise gaps in service delivery.\u00a0<\/span><\/li>\n<li style=\"text-align: justify;\"><span style=\"font-weight: 400;\">PM-FCT is designed as a<\/span><b> family-centric digital platform<\/b><span style=\"font-weight: 400;\">, rather than maintaining separate records for different schemes, allowing authorities to<\/span><b> track the health and welfare status of every family member through a single interface.\u00a0<\/b><\/li>\n<li style=\"text-align: justify;\"><span style=\"font-weight: 400;\">The initiative aims to<\/span><b> improve convergence across multiple government departments<\/b><span style=\"font-weight: 400;\"> by <\/span><b>integrating data from birth and death registration systems<\/b><span style=\"font-weight: 400;\">, and h<\/span><b>ealth, nutrition and education databases.\u00a0\u00a0<\/b><\/li>\n<li style=\"text-align: justify;\"><span style=\"font-weight: 400;\">The <\/span><b>pilot project<\/b><span style=\"font-weight: 400;\"> will initially be implemented in <\/span><b>Gandhinagar <\/b><span style=\"font-weight: 400;\">and, if successful, is <\/span><b>proposed <\/b><span style=\"font-weight:
