Besides saving lives, tissue donation helps restore vision, repair damaged tissues and improve the quality of life of recipients.<\/span><\/p>\n<h3><b>Types of Organ Donation<\/b><\/h3>\n<p><span style=\"font-weight: 400;\">Organ donation is broadly classified into three categories.<\/span><\/p>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><b>Living Organ Donation<\/b><span style=\"font-weight: 400;\">: A healthy individual voluntarily donates one kidney or part of the liver and, in selected cases, part of the lung, pancreas or intestine.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><b>Deceased Organ Donation<\/b><span style=\"font-weight: 400;\">: Organs are donated after a person is declared brain-stem dead in accordance with prescribed medical and legal procedures. One deceased donor can potentially save several lives through transplantation of multiple organs.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><b>Tissue Donation<\/b><span style=\"font-weight: 400;\">: Tissues can be donated after death even when organ donation is not possible. Common tissues donated include corneas, skin, bones, tendons and heart valves.<\/span><\/li>\n<\/ul>\n<h3><b>Organs and Tissues That Can Be Donated<\/b><\/h3>\n<p><span style=\"font-weight: 400;\">Several organs and tissues can be transplanted to save or improve lives.<\/span><\/p>\n<p><span style=\"font-weight: 400;\"><strong>Organs<\/strong>: <\/span><span style=\"font-weight: 400;\">Heart, <\/span><span style=\"font-weight: 400;\">Kidneys, <\/span><span style=\"font-weight: 400;\">Liver, <\/span><span style=\"font-weight: 400;\">Lungs, <\/span><span style=\"font-weight: 400;\">Pancreas, <\/span><span style=\"font-weight: 400;\">Intestine.<\/span><\/p>\n<p><span style=\"font-weight: 400;\"><strong>Tissues<\/strong>: <\/span><span style=\"font-weight: 400;\">Corneas, <\/span><span style=\"font-weight: 400;\">Skin, <\/span><span style=\"font-weight: 400;\">Bone, <\/span><span style=\"font-weight: 400;\">Tendons, <\/span><span style=\"font-weight: 400;\">Heart valves<\/span><\/p>\n<h2><b>Organ Donation in India<\/b><\/h2>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">India performed <strong>over 20,000 organ transplants in 2025<\/strong>, crossing the milestone for the first time and marking nearly a four-fold increase from fewer than 5,000 transplants in 2013.\u00a0<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">According to NOTTO, around 20,019 organ transplants were performed in 2025.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">As of 3 March 2026, nearly 89,839 patients were on the national waiting list for major organ transplants, highlighting the persistent gap between demand and supply.\u00a0<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">India has recorded more than 5 lakh organ donation pledges, reflecting growing public awareness and participation in voluntary donation.\u00a0<\/span><\/li>\n<\/ul>\n<p><span style=\"font-weight: 400;\">Despite this progress, <strong>deceased donor organs account for only around 18% of all organ transplants<\/strong>, indicating India&#8217;s continued dependence on living donors.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">India has established a comprehensive legal and institutional framework to promote ethical organ donation, regulate transplantation and prevent commercial exploitation while ensuring equitable access to transplant services.<\/span><\/p>\n<h3><b>Legal Framework<\/b><\/h3>\n<p><span style=\"font-weight: 400;\">The <\/span><b>Transplantation of Human Organs and Tissues Act (THOTA), 1994<\/b><span style=\"font-weight: 400;\"> is the principal legislation governing organ donation and transplantation in India.<\/span><\/p>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">It regulates the removal, storage and transplantation of human organs and tissues for therapeutic purposes.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">It legally recognises <\/span><b>brain-stem death<\/b><span style=\"font-weight: 400;\">, enabling deceased organ donation.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">It prohibits the commercial trade of human organs and prescribes penalties for organ trafficking.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">It provides procedures for living and deceased organ donation based on informed consent.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">The <\/span><b>Transplantation of Human Organs and Tissues (Amendment) Act, 2011<\/b><span style=\"font-weight: 400;\"> expanded the law to include tissues, strengthened safeguards against commercialisation and simplified organ donation procedures.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">The <\/span><b>Transplantation of Human Organs and Tissues Rules, 2014 <\/b><span style=\"font-weight: 400;\">prescribe operational guidelines for hospitals, transplant centres and authorisation committees<\/span><\/li>\n<\/ul>\n<h3><b>Institutional Framework<\/b><\/h3>\n<p><span style=\"font-weight: 400;\">India follows a t<\/span><b>hree-tier institutional mechanism<\/b><span style=\"font-weight: 400;\"> for coordinating organ donation and transplantation.<\/span><\/p>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><b>National Organ and Tissue Transplant Organisation (NOTTO) <\/b><span style=\"font-weight: 400;\">functions as the apex body under the Ministry of Health and Family Welfare for policy coordination, the national waiting list, organ allocation and awareness.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><b>Regional Organ and Tissue Transplant Organisations (ROTTOs) <\/b><span style=\"font-weight: 400;\">coordinate organ sharing among groups of States and Union Territories.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><b>State Organ and Tissue Transplant Organisations (SOTTOs) <\/b><span style=\"font-weight: 400;\">manage organ donation, allocation and awareness programmes within individual States.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">India also observes <\/span><b>National Organ Donation Day on 3 August <\/b><span style=\"font-weight: 400;\">to promote awareness and recognise the contribution of organ donors.<\/span><\/li>\n<\/ul>\n<h2><b>National Organ Transplant Programme (NOTP)<\/b><\/h2>\n<p><span style=\"font-weight: 400;\">The Government of India implements the <strong>National Organ Transplant Programme (NOTP)<\/strong> to strengthen organ donation and transplantation services across the country.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Its major objectives include:<\/span><\/p>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Strengthening transplant infrastructure in government hospitals.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Establishing NOTTO, ROTTOs and SOTTOs.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Maintaining a national waiting list and transparent organ allocation system.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span social challenges.<\/span><\/p>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><b>Huge demand-supply gap<\/b><span style=\"font-weight: 400;\">: Nearly 90,000 patients are waiting for organ transplants, while only a fraction receive organs each year.\u00a0<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><b>Low deceased organ donation<\/b><span style=\"font-weight: 400;\">: Most transplants rely on living donors, whereas deceased donation remains underutilised.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><b>Limited awareness of brain-stem deat<\/b><span style=\"font-weight: 400;\">h: Many families are unfamiliar with the legal concept of brain-stem death, resulting in refusal to donate.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><b>Family consent barriers<\/b><span style=\"font-weight: 400;\">: Even when individuals pledge their organs, transplantation often depends on family consent after death.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><b>Regional disparities<\/b><span style=\"font-weight: 400;\">: States such as Tamil Nadu, Telangana, Maharashtra, Karnataka and Gujarat perform substantially better than many other States due to stronger institutional networks.\u00a0<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><b>Infrastructure constraints<\/b><span style=\"font-weight: 400;\">: Many districts lack organ retrieval centres, transplant coordinators and intensive care facilities.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><b>Data reporting gaps<\/b><span style=\"font-weight: 400;\">: In 2025, 217 of 804 registered transplant hospitals failed to upload mandatory transplant data to the National Registry, affecting monitoring and transparency.\u00a0<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><b>Illegal organ trafficking<\/b><span style=\"font-weight: 400;\">: Preventing commercial exploitation and ensuring ethical transplantation remain continuing regulatory challenges.<\/span><\/li>\n<\/ul>\n<h2><b>Way Forward<\/b><\/h2>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Increase deceased organ donation by improving awareness about brain-stem death.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Integrate organ donation counselling into intensive care units of major hospitals.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Strengthen NOTTO, ROTTOs and SOTTOs through greater financial and technical support.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Improve digital integration of transplant waiting lists and organ allocation systems.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Ensure timely reporting of transplant data by all registered hospitals.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Expand organ retrieval centres and green corridor mechanisms for rapid organ transport.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight:
