The WHO classifies it into undernutrition - including wasting, stunting, underweight and micronutrient deficiencies and overweight, obesity and diet-related non-communicable diseases.

In India, malnutrition reflects the interaction of inadequate diets, infections, maternal and child health, poverty, sanitation, feeding practices and unequal access to nutrition services. Although nutritional outcomes have improved over successive NFHS rounds, persistent child undernutrition coexists with rising overweight and obesity, creating a double burden of malnutrition. Addressing it therefore requires a lifecycle approach combining food security, dietary diversity, healthcare, WASH, social protection, behavioural change and effective last-mile delivery.

Malnutrition Meaning

Malnutrition refers to deficiencies, excesses or imbalances in the intake or utilisation of energy and nutrients.

According to WHO, it includes undernutrition, micronutrient-related malnutrition, overweight, obesity and diet-related non-communicable diseases, thereby encompassing both inadequate and excessive nutrition.

The coexistence of undernutrition with overweight, obesity or diet-related NCDs is known as the double burden of malnutrition.

Malnutrition Types

Malnutrition encompasses deficiencies, excesses or imbalances of energy and nutrients. It can therefore manifest as undernutrition, overnutrition or specific protein-energy and micronutrient disorders.

Undernutrition: It comprises wasting, stunting, underweight and micronutrient deficiencies.

Wasting refers to low weight-for-height and generally indicates acute or recent undernutrition, often associated with illness or inadequate food intake.

Stunting means low height-for-age and reflects chronic or recurrent undernutrition, which can impair physical and cognitive development.

Underweight refers to low weight-for-age and may result from stunting, wasting or both.

Micronutrient deficiencies involve inadequate vitamins and minerals, such as iron, vitamin A and iodine.

Overnutrition: It results from excessive intake or accumulation of nutrients.

Macronutrient overnutrition may cause overweight and obesity, increasing the risk of diabetes, cardiovascular diseases and other NCDs, while micronutrient excess may result from excessive or inappropriate supplementation.

Protein-Energy Malnutrition: Severe forms include Kwashiorkor, characterised by protein deficiency and oedema, and Marasmus, characterised by severe energy deficiency and marked wasting.

Malnutrition Causes

Malnutrition in India is multidimensional, arising from the interaction of immediate nutritional deficiencies with household, social, environmental, health and institutional factors. Its causes can be broadly grouped into direct causes and underlying structural causes.

Direct Causes of Malnutrition

Inadequate dietary intake: Insufficient quantity, poor dietary diversity and inadequate consumption of protein- and micronutrient-rich foods can lead to undernutrition.

Disease and infections: Recurrent diarrhoea and other infections can reduce appetite, impair nutrient absorption and increase nutrient requirements, creating a vicious cycle between illness and malnutrition.

Poor infant and young child feeding practices: Inadequate breastfeeding, delayed initiation of complementary feeding and poor-quality complementary diets can adversely affect child nutrition and development.

Maternal undernutrition: Poor maternal nutrition can contribute to low birth weight and increase the risk of malnutrition across generations.

Underlying and Structural Causes of Malnutrition

Poverty and food insecurity: Low household income and inadequate access to nutritious food restrict dietary diversity and increase vulnerability to malnutrition.

Poor WASH conditions: Unsafe drinking water, inadequate sanitation and poor hygiene increase exposure to infections and can undermine nutritional gains.

Gender and social inequalities: Unequal access to food, healthcare, education and household resources can disproportionately affect women and children.

Weak last-mile delivery: Gaps in Anganwadi infrastructure, service quality, supply chains, monitoring and outreach can reduce the effectiveness of nutrition programmes.

Limited awareness: Inadequate knowledge of breastfeeding, complementary feeding, dietary diversity, hygiene and appropriate nutrition practices can contribute to poor outcomes.

Environmental and health factors: Soil-transmitted helminth infections, recurrent illnesses and other environmental health risks can impair nutrient absorption and aggravate nutritional deficiencies.

Malnutrition in India

India has made significant progress in reducing several forms of child undernutrition, but the nutritional burden remains substantial and uneven across population groups.

NFHS-6 (2023–24): The prevalence of stunting among under-five children declined to 29.3%, while underweight declined marginally from 32.1% to 31.8%.

Severe wasting (too thin for one’s height) declined significantly from 7.7% to 5.2%.

NFHS-6 also reported improvements in infant and young child feeding practices, with children aged 6–8 months receiving solid or semi-solid food along with breastmilk increasing from 45.9% to 59.5%.

Anaemia: NFHS-5 reported anaemia among 67.1% of children aged 6–59 months, 57.0% of women aged 15–49 years and 52.2% of pregnant women.

The discontinuation of comparable anaemia estimates in NFHS-6 creates a data-continuity challenge for monitoring progress.

Double burden: India increasingly faces the simultaneous challenge of persistent undernutrition and rising overweight, obesity and diet-related NCDs, requiring nutrition policies to address both ends of the spectrum.

NFHS-6 reports that 30.7% of women and 27.3% of men aged 15–49 years were overweight or obese.

<span Forward

India's nutrition strategy must move from a calorie-centric approach towards nutrition security and dietary diversity.

Strengthen the first 1,000 days: Prioritise maternal nutrition, breastfeeding, complementary feeding and early childhood interventions.

Improve dietary diversity: Promote affordable access to protein-rich foods, fruits, vegetables, pulses, millets and micronutrient-rich local foods.

Adopt a lifecycle approach: Integrate maternal, adolescent, child and elderly nutrition rather than treating child malnutrition in isolation.

Strengthen WASH convergence: Combine nutrition interventions with safe water, sanitation, hygiene and infection prevention.

Improve last-mile delivery: Address Anganwadi infrastructure, frontline-worker capacity, supply-chain gaps and beneficiary exclusion.

Use technology responsibly: Strengthen Poshan Tracker-based monitoring while ensuring data quality, privacy and independent verification.

Improve nutrition data: Maintain continuity and methodological consistency in national nutrition surveys so that policy progress can be assessed reliably.

Address the double burden: Nutrition policy must simultaneously tackle stunting, wasting and micronutrient deficiencies alongside overweight, obesity and diet-related NCDs.

Promote community ownership: Panchayats, women's groups, schools, SHGs and civil society should be integrated into local nutrition planning and accountability.

Malnutrition UPSC PYQs

Q1. Malnutrition in
